Humana Changes for 2027: Your County Matters

Start with the letter—not the headline.

Humana continues to offer Utah coverage, but some plans and service areas are changing. The right next step depends on your exact plan number and county. Greg & Adele can help you review the difference between a renewal, a crosswalk, and coverage ending in your area.

Schedule Your Humana Plan Review

Humana is still offering plans in Utah

Headlines about national Medicare Advantage exits do not mean Humana is leaving every Utah county. Humana's October 2026 announcement describes its 2027 offerings and expanded specialized coverage in Utah. CMS's 2027 listings also show continuing Humana options in the state.

The question to answer is more specific: What happens to your exact plan in your county? A non-renewal, a smaller service area, and a renewal under another plan number can have different consequences. Your notice is the starting point.

Value Choice H5216-456: a plan-number and service-area change

The 2026 plan Humana Value Choice H5216-456 (PPO) no longer appears under that number in the 2027 landscape. CMS's crosswalk maps it to H5216-471, Humana Value Choice (PPO), as a renewal with a service-area reduction.

Where H5216-471 is listed

Cache, Davis, Morgan, Rich, Salt Lake, Utah, and Weber counties. If you live in one of these counties, review your Humana renewal documents to confirm the new plan, benefits, and enrollment status.

Former H5216-456 counties outside that listing

Iron, Summit, Tooele, Wasatch, and Washington. The mapped H5216-471 plan is not listed in these counties. Read your notice and review other coverage options; this is not a statement that all Humana plans leave these counties.

The county makes a difference: some Humana members can be crosswalked to another plan in counties such as Cache. The H5216-456 to H5216-471 crosswalk does not provide a replacement in Washington or Iron County. Affected H5216-456 members there need to review other 2027 coverage choices with Greg & Adele.

Do not rely on the words “Value Choice” alone. Other plans can use similar names. Compare the full contract-plan number on your letter, as well as its doctors, prescriptions, costs, and benefits.

St. George and Washington County still have choices

CMS lists several Humana plans in Washington County for 2027, along with plans from other carriers. Some have $0 monthly plan premiums. That does not make all care free: Medicare premiums, copays, deductibles, and other costs can still apply.

Greg & Adele can help Washington County and Iron County residents review what is available locally. We also help clients across Utah compare choices based on their own county and situation.

Turn your notice into a coverage plan

  1. Identify the type of change

Bring the Annual Notice of Change, renewal information, or non-renewal letter. We can help separate a routine benefit update from a plan ending in your area.

  2. Check the services you use

List your doctors, hospitals, medications, doses, and pharmacy. Review network participation and prescription costs for 2027, even if you stay with Humana.

  3. Compare costs and alternatives

Review the monthly premium, drug deductible, copays, and annual medical out-of-pocket limit. If you need a replacement, compare remaining Humana plans, other Medicare Advantage plans, and Original Medicare with separate drug coverage and, when appropriate, Medigap.

  4. Confirm what happens January 1

Know whether you are renewing, need to enroll in a replacement, or will return to Original Medicare. Confirm any application acceptance and effective date. We can help explain the steps so you are ready for the new year.

Which enrollment dates apply?

  • October 15–December 7, 2026: Annual Enrollment

Review or change Medicare Advantage and Part D coverage for January 1, 2027. The plan must receive your enrollment request by December 7.

  • December 8, 2026–February 28, 2027: qualifying non-renewals

Medicare offers this special enrollment window when a plan's contract is not renewed. Confirm whether your particular change qualifies and when new coverage would begin.

  • Medigap rights depend on the actual change

A qualifying plan exit or loss of service in your area may provide guaranteed-issue rights when you return to Original Medicare. The application period generally runs from 60 days before to 63 days after coverage ends. For a December 31, 2026 end date, that is November 1, 2026–March 4, 2027.

A new plan number alone does not establish non-renewal rights. We can help review your notice before you rely on a special enrollment period or Medigap buying protection.

What if the letter says your coverage is ending?

If your plan is truly non-renewed and you do not join another Medicare Advantage plan before it ends, Medicare says you will return to Original Medicare. Parts A and B do not include a stand-alone Part D drug plan or an annual limit on medical out-of-pocket spending without supplemental coverage.

A renewal into a successor plan is a different situation. Make sure you know which applies to you rather than making a decision from a national headline.
